Patient Information

At Headaches and Pain Care Center, you are the most important member of the treatment team. Your experience and observations of your medical condition will help contribute to your care and provide our clinicians with information for your personalized treatment plan. Our responsibility to you is our dedication to establishing a innovative health care model that enhances your life and your well-being. We want you to understand the importance of taking an active role in the treatment of your headache.

Your care begins with your first phone call to our facility. When you call, or when your referring doctor calls, the friendly receptionist will get the necessary registration information. You will be asked a few questions regarding your reason to visit our office.

Once scheduled, you will be contacted by phone prior to your arrival time and date. You will also be encouraged to sign up on patient portal on this page which will offer you access to patient forms and medical informations. It will also contain papers that you will need to fill out and bring with you on the day of your appointment. We would like to advise you to come at least 25 minutes ahead of your schedule time to allow enough time to fill out patient information papers.

Once the clinician has examined you and assessed your situation, your treatment plan will be developed with your assistance. You may be required to follow-up with other medical tests, Some lifestyle management or schedule a consultation with a specialist or therapist. Whatever the recommendation is, it is made by Dr Patel with the belief that it will make a positive difference to the outcome of your medical condition.

Overall, our goal is to provide you with a comprehensive, inclusive and integrated approach to your medical condition. And to succeed, we need you as an active participant in your treatment. We need you to follow up with your appointments and to comply with medication plans and lifestyle recommendations. We are only successful as a team if it includes you.

Insurance:

Headaches and pain Care Center is an independent facility and is in-network provider for major Insurance companies in the state of Indiana. Currently our clinic participate in-network with insurance companies like Medicare, Blue Cross/Blue Shield, Anthem silver, gold and platinum through Obamacare, Aetna, Cigna, United Health, UMR, Indiana Carpenters and Tricare/Triwest. Currently we are not a participant provider for Medicaid, Hoosier health wise, Anthem HIPP or MD wise state program. Our billing department will verify insurance benefits and inform you if your benefits are out of network. Insurance claims are filed as a courtesy for our patients. Some procedures performed at our center are not covered by insurance, and charges for those procedures are the responsibility of the patient.

Payment in full is expected at the time of service, including co-pay and co-insurance or past dues amounts.

Prior Authorization:

Prior Authorizations: If your insurance requires prior authorization or referrals from a family physician to see a provider at headaches and pain care center, it is your responsibility to obtain such authorization/referrals prior to the first appointment.

Our office staff will be happy to initiate the process of Prior Authorization (PA) for the Prescriptions written at our clinic in a timely manner, ultimately it will be your responsibility to follow up with your pharmacy or Insurance company in case of any delay regarding this process of Prior Authorization by Insurance.
